Telehealth Consent Notice
Effective Date: September 30, 2026
Telehealth allows a patient and licensed healthcare professional to communicate using electronic technology when they are in different locations.
Telehealth may involve electronic communications, medical history review, clinical evaluation, follow-up, and other healthcare services permitted by applicable law.
Potential benefits include convenient access to licensed healthcare professionals without an in-person visit. Telehealth also has limitations and risks, including limitations associated with remote evaluation, technology failures, interruptions, privacy and security risks, and circumstances in which an in-person examination or other care may be appropriate.
Participation in telehealth does not guarantee a diagnosis, prescription, medication, laboratory order, or particular treatment.
The treating licensed healthcare professional independently determines whether telehealth is appropriate and whether any treatment is medically necessary.
Prescription medications are prescribed only when a licensed healthcare professional determines they are medically appropriate and when permitted by applicable law.
Telehealth is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or seek immediate emergency care.
Additional consent language may be presented during clinical enrollment or consultation based on the provider, service, and patient's state.
